Job description:

New hope is a 24 hour CQC registered service that provides a well-coordinated blend of clinical and accommodation services. The service is for vulnerable male adults with a diagnosis of mental ill health, forensic history and may also have personality disorder and substance misuse issues.

All service users require a high level of support and close monitoring to reduce the factors that may have led to their previous mental health deterioration and offending behaviour.

Purpose of Role

To support service users to achieve their personal goals and positive outcomes.

To support Service users to identify and take positive risks, within a recovery model to enable service user move – on to greater independence and lead more fulfilling lives

Areas Of Responsibility

Work within the service’s policy and procedural framework established, ensuring full up to date knowledge and understanding of policies and procedures at all times.

Act as a key worker and a link worker for named service users and in this role contribute to the development of support plans, risk assessments and subsequent reviews for allocated service users.

Develop and sustain therapeutic relationships with service users within the agreed support plans.

Provide practical and emotional support to all service users of the Service, ensuring that they are treated with respect and dignity at all times

Deliver life skills training and support to prepare service users for successful resettlement into semi/independent living.

Maintain a high standard of service as required by the Commissioners and CQC in order to deliver a safe, effective, caring, responsive service and contribute to the service achieving a CQC outstanding rating.
